 Contre les menaces sur la santé et la protection sociale,

agissons ensemble!

Forum Social Mondial de Tunis, 28 Mars 2015

 

Sur base d'un projet élaboré depuis plusieurs semaines par une trentaine d'organisations, nous, mouvements sociaux, syndicats, collectifs et individus présents à Tunis pour le Forum Social Mondial, avons partagé nos analyses, nos expériences et nos perspectives autour de la santé et de la protection sociale.

 

Nous avons abordé les questions liées aux déterminants sociaux de la santé comme la pauvreté, les conditions de travail, l'éducation, l'inégalité de genre ou encore l'accès à l'eau. Nous avons également discuté de l'augmentation croissante de la marchandisation de la santé et de la protection sociale.

 

Ces discussions ont permis de constater que la crise de la santé et de la protection sociale est en fait la conséquence des politiques néolibérales globales:

·         La financiarisation de l'économie, aidée par les instances monétaires internationales, et l'endettement engloutissent toutes les nations, grandes et petites, imposent l'austérité et promeuvent les intérêts des banques et des multinationales au détriment des politiques sociales et sanitaires.

·         Des rapports de force déséquilibrés se traduisent par des traités de libre échange favorisant les bénéfices des banques et des multinationales au détriment des peuples, avec la complicité de nombreux gouvernements.

·         La globalisation du marché de la santé et de la protection sociale a des répercussions catastrophiques sur l'accès à la santé, les emplois, les retraites, les conditions de travail, la qualité de la prise en charge et la migration des travailleurs de la santé du sud vers le nord du monde et du secteur public vers le secteur privé.

·         La croissance de la militarisation et l'occupation de territoires provoquent morts, pertes de terres, de travail et de nourriture; elles provoquent également la montée de l'intolérance entre communautés, le terrorisme et les conflits sectaires.

·         Le plus grand fardeau de la crise est porté par les plus fragilisés – femmes, enfants, migrante-s, pauvres, personnes en situation de handicap, travailleurs/euses et paysan-ne-s.

 

La santé est la vie dans toutes ses dimensions: physique, mentale, sociale, environnementale. Elle est un droit humain fondamental et inaliénable pour tous et toutes et un bien social commun de toute l'humanité; elle présuppose de permettre à la population de vivre en paix partout dans le monde et hors de toute occupation.

 

Il y a urgence, des alternatives s'imposent!

·         Les droits des populations en matière d’environnement, d'emploi, de conditions de travail, d’accès à l’eau, d’éducation, d'alimentation, de culture, de logement, d’accès au bien-être, doivent être mis en œuvre, garantis, défendus et étendus.

·         Une protection sociale universelle et globale doit être conçue et mise en oeuvre pour promouvoir la justice sociale et la dignité.

·         La priorité doit être donnée aux soins de proximité et à la santé communautaire, incluant la prévention et l’utilisation respectueuse des ressources naturelles.

·         Un système unifié de santé et de protection sociale doit être entièrement public et basé sur une taxation nationale progressive - entre autres du capital - et/ou des cotisations sociales.

·         Un tel système doit appartenir à tous et a toutes et permettre à la population de le contrôler et d’y participer pleinement.

·         Ce système doit se situer hors de la logique marchande et doit garantir un accès aux soins gratuit.

·         Il s’agit de garantir le droit d'accès aux médicaments nécessaires, de bonne qualité et non protégés par des monopoles de droit intellectuel.

·         Les instances sanitaires internationales doivent être transparentes et totalement indépendantes des intérêts des multinationales et des financements privés.

 

 

Forts de nos expériences réussies, passons à l’action!

·         Travaillons en réseau, faisons circuler la connaissance et les analyses, élargissons nos mouvements, nourrissons-nous de nos mobilisations respectives et créons de nouvelles solidarités internationales permettant d’amplifier les rapports de force.

·         A partir des réalités spécifiques locales, sectorielles, conjoncturelles... démontons les mécanismes, rendons accessibles les analyses et les outils pédagogiques, renforçons et faisons converger les mouvements et capacités d’actions. Participons au développement de la capacité d’analyse politique sur les choix de société.

·         Agissons sur la sphère politique:

◦     les lois doivent garantir le droit effectif à la santé et à la protection sociale;

◦     créons des mouvements de base suffisamment forts pour exercer un contrôle et une pression sur le politique pour qu’il respecte et rende effectifs ces droits.

·         Professionnels, usagers, citoyens... devenons des acteurs de changement par la formation et la sensibilisation.

·         Créons des alliances entre les professionnels et les usagers, entre les syndicats et les mouvements «citoyens», et favorisons l’émergence d’alliances locales multiformes en défense de la santé et la protection sociale.

·         Renforçons les actions par des convergences avec les mouvements agissant sur les déterminants de la santé, tels que le climat, le commerce, l’austérité, la dette, les conditions de travail, l’égalité hommes/femmes….

Against all threats to health and social protection,

let's mobilise together!

 

World Social Forum - Tunis, 28 March 2015

 

 

Starting from a proposal issued by an initial group, we as organisations, social movements, trade unions, and other groups and individuals present in Tunis for the World Social Forum shared our analysis, experiences and perspectives around health and social protection.

 

We focused on issues related to the social determinants of health such as poverty, working conditions, education, gender inequality, and access to water. We also discussed the growing marketisation of health, healthcare and social protection worldwide.

 

Our discussions show that the crisis in health and social protection is in fact the consequence of the global neoliberal politics:

 

·         Financialisation of the economy, supported by monetary policies, as well as international debt engulf all nations, big and small, forcing austerity on the people and promoting interests of banks and multinational companies to the detriment of social and health policies.

·         Unequal power relations translate into trade treaties that place profits of banks and multinational companies over those of people, with the complicity of several governments.

·         A globalised marked for health care and social protection has catastrophic impact on access to services, employment, retirement, working conditions, the quality of care and migration of health workers from the global South to the North and from the public to the private sector.

·         Increased militarisation and occupation cause death, loss of land, work and food, while also promoting a reaction in the form of rising intolerance between communities, terrorism and sectarian conflicts.

·         The major burden of the crisis is faced by those already marginalised – women, children, migrants, the poor, people living with disabilities, workers and peasants.

 

Health is life in all its dimensions: physical, mental, social, environmental. It is a fundamental and inalienable human right and a common social good for all humanity; it requires that people can live in peace all over in the world and free from occupation.

 

Alternatives are urgent and necessary!

 

·         The rights of people to clean environment, good employment and working conditions, access to water, education, food, culture, housing and well being must be realised, granted, defended and expanded.

·         A universal and comprehensive social protection designed to promote social justice and dignity needs to be put in place.

·         Priority must be given to an approach based on the principles of comprehensive primary health care, including prevention and the respectful use of natural resources.

·         A unified health and social protection system must be entirely public, financed by progressive taxation – including of capital – and/or social contributions.

·         This system must be owned by the public and accountable to the public through participatory systems of governance at all levels.

·         It must reject all forms of marketisation and be free at the point of care.

·         The right to access to essential medicines, of good quality and not protected by patent monopolies must be granted.

·         International institutions must be transparent and totally independent from interests of multinationals and private financing.

 

Inspired by our experiences, we believe that the time is now for collective action!

 

·         Let us work through our networks, sharing and circulating knowledge and analysis, expanding our movements, strengthening one another in our mobilisations and creating new international solidarities that can help us change the power balance.

·         Starting from the concrete local, sectoral and conjunctural experiences, let us popularise our analysis and make the pedagogic tools accessible, strengthening our convergences and increasing our capacity of action. Let us participate in the development of the capacity of political analysis on the choices before society.

·         Let us act on the political level:

◦     laws must guarantee a real right to health and social protection;

◦     let us create strong popular movements that can control and put pressure on political sytems so that they respect and enact these rights.

·         As health professionals, users, citizens... Let us become, through training and sensitisation, true agents of change.

·         Let us create alliances between professionals and users, trade unions and citizen movements, and let us promote local alliances among diverse actors in defence of health and social protection.

·         Let us strengthen the actions for a convergence with movements acting on social determinants of health, such as climate, trade, austerity, debt, working conditions, and gender equality.
